Are these forms standard with a modeling contract? Login/Join

Hi everyone
complete newbie to the business. I got offered to be represented by an agency. By my research on this helpful forum, they seem to be a reputable agency.

They are asking for a W-9 tax form filled out and a Check Authorization form filled out. Are these standard forms that all of you filled out?

The check authorization form seems kind of scary ... I am no lawyer, but it seems like they could cash ANY check made out to my name with this signed? And it doesn't appear to have an "end" date.

Thanks

Yes, it's standard for them to have check authorization. They receive the checks, deposit them, then cut you a cut minus their percentage... Yes, they will cash all your checks that are filled out to you - they get their cut before you get yours...

You'll get a 1099 from them for your taxes, you are not considered an employee.

I wrote in an end date that matched the contract end date.
