Are you looking for a modeling permit (print) or a child performer (acting permit)? NY state has a new form (as of 6/7) for the child performer permits. Here's the link: http://www.labor.state.ny.us/formsdocs/wp/LS561.pdfLet me know if you are looking for info on the modeling permit. I have it somewhere...I'm sure others on this board might have it at their fingertips.

Yes, thanks for posting that...that is how it is done in NYS. The form is different for CA, which is where Allyb is applying.

When I was homeschooling my son in CA, I had to go to the district office for them to sign and stamp the permit. The labor board would not accept my own signature as his teacher, even though I had an official school name with an affidavit. In CA, the student and the parent must go to your local school district office to have it signed and stamped, unless you are affiliated with another homeschooling program such as On Location Education or Laurel Springs, for example. Hope that helps!

Yes, that would be a dream come true for all parents of child actors. But, since each state has its own laws and state regulations, that will probably never happen. Well, at least not in my lifetime!LOL

Curious, would you get a work permit for your child before they start working or once you have booked them in anticipation of work?
