They left my "professional name if different" blank. But i sent the application with a name in that section!

Do I send it back, wait six months, write it in the section with a pen.

Please help!

Also for my birth last name my name was put together. My last name is Del Rio, they put it as delrio.

How do I change these to things.

When you book a role do they even look at this for the name or do they just use your birth certificate and SSN. Please help!!!

Just make the corrections in 6 months. When you book use whichever name you want to be credited with. However, if you book a paying gig make sure they pay you in the name of your trust account.
I noticed at work that pay checks were sent out in real names as opposed to stage names in some cases.

We have always received the form with the "professional name if different" area blank. I fill it in myself.

DadinWestchester, I haven't booked a role yet. But I did recently audition for a movie. I'm not too sure how the process works.

What happens when you book a role? Do you have to fill out papers and stuff? I just want to make sure that I get to choose the name i get to be credited for.

Somuchtodo, Yeah, I might just fill it in myself. Does that make it not valid though?

What I meant is, I want to make sure that being that the permit is blank would that effect the name I am credited for? If that makes sense.

Once you book a role that pays you will have to fill out paperwork. Have a copy of your performer permit and trust account info. If it is a union job they will want to keep track too. You are only allowed so many union gigs before you have to join.
If the job is non paying, they won't ask for anything.(Most of the time)
