It has always been requested/suggested to me by various agents to place the name on the bottom right, as most (but not all) agents logos go on the bottom left. This also allows the actor's name to be easily visible when agents or CDs thumb through the photos or turn pages of headshots after they have been filed into notebooks or files.

Oh boy, staples!  Regardless of 2, 3 or 4 staples, if you staple directly on the line of the border, you don't see the staples and it doesn't interfere with the name at all.

Thats if you use an adjustable stapler. Actually, since I do so much headshot mailings, I have years ago, purchased a Powered, Flat Clinch stapler. The results are staples that are FLAT to the paper rather then the double curled standard staple. this is better for the CD as they dont poke fingers. and flatter is nicer. and its alot less effort. The machine costed 160 dollars but It was very well worth the money. I have used it for years, but the only thing is, you cant adjust the depth of the staple, it goes on a 1/4 inch from the edge and thats that. and I find that I cant REACH the border line with it so I avoid stapling near the name.

The name ALWAYS goes at the bottom, usually off to the right. But that depends on the actual shot and how the head is framed in the picture. I've seen the name on either side. There's no set rule about this. Keep in mind that most of that will be cut off for electronic submissions but you're right to follow standard operating procedure for hard copy submissions.

NEVER HANDWRITE ANYTHING! Even if it is the resume. Just know from now on to have your next batch with your name printed somewhere on the bottom. I think the best thing you can do with your mistake is use those Avery labels (maybe those clear ones) or put an avery label with your name and cell phone number on the back of the headshot in case your resume comes off.
