-Yes you should have headshots that you use for commercials and other headshots for theatrical. Sometimes you can use your commercial headshots for theatrical, but not the theatrical for commercials.
-What do you look like? It's your choice if you want to do commercial print. It's good short term money. Most commercial agents handle commercials, industrials, and commercial print.
However, there are no unions for commercial print so you can actually have more than one commercial print agent, granted that you don't sign some exclusivity agreement. Of course give the commission to whomever notified you of the audition first.
A commercial headshot is fine when starting out for commercial print. Over time, you can put together a comp card. You can also join model mayhem or look for budding photographers who will trade for print. Even at commercial print auditions, I hardly ever see people with comp cards these days.
