If the physical challenge is immediately apparent, yes. I'd include it. I shot a woman last year who was completely blind, and it generated her lots of interest. Good question. The first line on a resume should always be your name, the second line your union affiliations (if any), then your contact information and your physical characteristics (hair and eye color, height and weight).
